What happened PLD-COIL TUBING PARTS WHILE
PULLING OUT OF WELLBORE A coil tubing (CT)
operation had completed drilling out plugs,
circulating the well bore and started pulling out
of the hole. After pulling out approximately 9
ft., the tubing parted at the surface just
outside of the gooseneck. The parted end of the
2 tubing on the spool side of the break recoiled
towards the spool and struck the operators cab
visor and windshield, cracking the safety glass.
The well was immediately secured per established
procedures. A buffer zone was in effect and the
immediate area was clear of personnel.
Investigation is currently underway.

  • Hazard Identification - During JSA and pre-job
    safety meetings, hazards and mitigation steps
    were identified and communicated to all involved
    site personnel.
  • Buffer Zone was in effect - clear of personnel
    Before travelling in or out of well bore, buffer
    zones around the Coil Tubing unit and wellhead
    were established and enforced.
  • Coil Tubing Integrity - All elements pertaining
    to the condition of the pipe must be considered
    before the job is started (CT reel log time in
    service, gas service, fluid pumped, well bore
    fluids exposure, etc.)
  • Tubing on Reel must be secured Procedures and
    necessary equipment/tools should be available to
    secure the reel end of the tubing should a
    similar failure occur to prevent additional
    spinning or flaring out of reeled tubing.
