- an Application

2A security feature has been added to the
application. To access the application, you will
need to contact Lil Emerson (271-2657) to set up
your password. You will also need to indicate the
District(s) you will be submitting.
3This is the first application screen.
4Project Specifics there will be some pre-filled
fields. Select EDIT to finish these sections.
5You must answer YES or NO to the District PD
Master Plan question.
6Participation include those participants in
Title II-A funded HQPD activities. DO NOT count
an individual more than once even if that person
will be participating in multiple HQPD
activities.Be sure to enter the number of
teachers who have received or will receive any
high quality professional development regardless
of the funding source between July 1 and June 30
of the current year.
7You will be given prompts if you enter invalid
data.Click the SAVE button when you are done.
8Activities/Expense select this link to add or
modify the activities and related expenses.
9This is the Activity application screen.
10Type of Activity, Category, Participants, and
School Type these are self-explanatory. NOTE
you may select more than one type of Participant
by holding down the CONTROL key (for MAC users
this is the APPLE key) and clicking on those
you wish to select.NEW you must provide an
answer.
11Purpose and Outcome these fields were changed
to dropdown boxes. NEW Description this
field requires a description of the activity and
the names of the nonpublic schools that are
involved. Outcome Data this field remains
the same.
12Click on ADD ACTIVITY after you have entered
the data for each activity. When you are
through entering all of the activities, click
on RETURN TO APPLICATION.
13After activities are completely entered, your
screen will look like this.
14Portions of the activities under HQPD Activities
and Purpose/Outcome may be edited. After you have
edited your information, click SAVE. If you
need to edit other information, you must DELETE
the activity and re-enter it.
15Add/Edit Expense this field is for the entry of
the dollar amounts specific to each activity.
16This is the Activity Expense application screen.
17Using the dropdown boxes, select the Expense
Category for each line item, key in the amount
using full dollars (000.00), the start and end
date of the activity (mm/dd/yy), and the
narrative for each line item. Click on ADD
EXPENSE for each line item. When you are through
with the expense for each activity, click on
RETURN TO ACTIVITIES.
18Add/Edit Participant this field is for the
entry of the participants specific to each
activity.
19This is the Activity Participants application
screen.
20Select the participants for the specific activity
you are working on. You may select more than one
by holding down the CONTROL key (for MAC users
this is the APPLE key) and making the
selections. When you are done click on ADD
PARTICIPANT and then click RETURN TO
ACTIVIES.To EDIT or DELETE an entry, click
the appropriate button.
21Add/Edit Class Size Reduction this field
applies ONLY to activities for Class Size
Reduction.
22This is the Reduce Class Size application screen.
23For Class Size Reduction activities ONLYFrom
the dropdown boxes select the appropriate School
and Grade(s) that will be taught (K
Kindergarten, M Multiple). Enter the Subject to
be taught and the Quantity of teachers. In the
FTE dropdown box, select the option that is
closest to meeting your need.When you are
through click on the ADD RCS button and then
click the RETURN TO ACTIVITY button.If you
need to EDIT or DELETE an entry, click on the
appropriate button.
24View/Add/Modify Explanation if HQPD activities
in the application do not assist teachers in
meeting the NCLB HQT requirement, an explanation
is necessary.
25This is the NCLB HQT Requirement application
screen.
26By selecting the ADD/EDIT button, the field opens
for your explanation. To SAVE your entry, click
on the appropriate button. When you are through
click on RETURN TO APPLICATION.If you need to
DELETE the entry, click on the appropriate
button.
27This is what the completed application will look
like.
28When all of the data entry is done, you must
click the SUBMIT button. A warning message
appears.
29To view applications that have been submitted,
there are four reports available.
