here for YOU.

  • PWS NAME
  • AI
  • SOURCE NAME
  • OPERATORS
  • COUNTY
  • DATE MAILED

UNIVERSAL eMOR FORM
Same for EVERY public water system in KY
There are no new regulations only a new form
5
ON the eMOR, (Excel file) these will be populated
throughout when you enter them on this cover page.
EVERY SYSTEM regardless of what type must have
these sections completed.
EVERY PUBLIC WATER SYSTEM
6
All eMOR forms and instructions are available for
you to download as Excel files (eMORs) and WORD
documents (instructions) at WWW.WATER.KY.GOV/DW
We encourage you to consult the KY Drinking Water
Branch if you need help. We are here for YOU.
7
  • Month and Year you are reporting for
  • Your PWSID (Be CAREFULLY correct)
  • PWS Name
  • Source Name County
  • Plant ID TREATMENT PLANTS ONLY

8
Plant ID FOR TREATMENT PLANTS ONLY
  • This is to indicate or identify which treatment
    plant the report is for.
  • Some systems have two TPs. (treatment plants)
  • Some have one TP that has been shut down and now
have another TP. (the old one-TPA and the new-
    TPB)
  • If left blank we assume that the report is for
    TPA.

9
  • Date Mailed
  • Superintendent/Operators

They do not need to sign their names on the MOR
- the person filling out the report may list the
names.
10
Next sectionTreatment Plants CompleteIf
you treat water, you are considered a treatment
plant.(excluding Cl boosters)
11
Those systems that treat water must complete this
section.
Percent Backwash Water Used - This section will
be automatically calculated from page 6 on the
Excel file when completing the eMOR on the
computer.
You cannot type the percentage in this field.
12
And FINALLY
Sign it and Date it
THIS should be the last step in the Monthly
Operating Reportafter all pages have been
completed and checked.
13

  • This column should coincide with the dates the TP
    was operating.

eMOR will total, average, and calculate for you
based on this column.
  • For dates the plant was not operating, leave that
    space blank.
  • Total gallons of raw water that were treated for
    the entire month.
  • Also include the hours of operation in the next
    column.

Complete/answer ANY yellow field on the eMOR
file..
14
eMOR will calculate for you the PPM based on
gallons of raw water treated and lbs. of chemical
added
  • Proceed through page 1 and page 2 to include any
chemicals added to the water by lbs. of actual
chemical - not the lbs of product.

gallons of product x 8.34 lbs. lbs. of
product Lbs. of product x chemical lbs. of
chemical
  • Include pounds of disinfectant added. - If only
one dose of disinfectant, use column for pre or
    post treatment.

Complete/answer ANY yellow field on the eMOR
file..

  • Page 3 and Page 5 - Water Quality
  • Page 3 - These are not required, but if you have
taken these you may provide the results on this
    page.
  • Chlorine residual here is different from the
distribution system residual on page 7.
  • This is for plant tap - not the entry point.

This column will NOT transfer to the Plant
Summary as the Entry Point Residual.
  • This turbidity reading should be taken before
water has been treated and/or gone through the
filter(s). These can be the average of multiple
readings or the lowest of the day.
  • Settled Water is the same as Top of Filter.

16
Lowest Daily Cl Residual Plant Tap. isthe
disinfectant residual at the entry point to the
distribution system
  • Every system that serves gt3,300 people (surface
water or groundwater under direct influence) are
required to have an on-line Chlorine analyzer.
  • This analyzer monitors continuously the residual
just before the water goes to the dist. system.
  • The number that belongs in this column for each
day is the lowest recorded residual for that day.

Do not forget to answer any field in yellow or
Y/N questions
This column is to be summarized on the Plant
Summary Sheet. The eMOR will populate it for you.
17
  • According to regulation, true groundwater systems
are not required to filter. Therefore, if you are
GW and you filter, we do not require you to
include this. If you have the information, it
would be helpful to include however.
  • The filter run hours have changed. It should be
the number of hours of plant operation each day
minus the number of hours the filter was off line
    for backwash.
  • 24 hours - 2 hr. (filter off-line) 22 Dont
total that with any hours from the next or
    previous day.

The eMOR will take this information, calculate
backwash percentage and populate the line on the
cover page. Else, you must calculate and include
on the cover page yourself.

Distribution System Test Results Chlorine
Residual
Check chlorine residuals DAILY at representative
points throughout the system. (401 KAR 8150
(1)(a)4)
  • The points used to sample within the
distribution system should represent the system.
  • At least one location must be sampled every day,
but the point(s) and the frequency you sample at
each should be decided upon based on the
    distribution system.
  • Chlorine boosters are not required Unless you
have them. If more than two, use the Additional
Data page 11 for the boosters.

Dont forget the yellow!
eMOR program will calculate totals at the bottom
and populate the section on Summary Sheet. If by
hand - no requirement to complete the totals at
bottom, but you will need to complete the Summary
Page section.
19
The eMOR will already have the Hours of Operation
and the number of samples required - calculated
just as the formula in the past. Enter the
turbidity results and the eMOR will list the
Maximum each day, count samples, and populate the
Plant Summary Form section.
20
Plant Summary Sheet and Summary Sheet Must be
completed and included by ALL WATER SYSTEMS
21
If you individually monitor filters, complete
these Y/N questions every month. (Only include
Page 9 if there was an exceedance.)
Summary of column on page 5 - the lowest Cl at
entry point

disinfectant.

this section.

  • The PWSID of the system to which you have sold
    water (including state ID)
  • The total amount sold to each system
  • Your own PWSID does not belong in this section
