JSA FORM MUST BE FILLED OUT PRIOR TO COMMENCEMENT

ASBESTOS OR LEAD -Jobs that require us to work
with asbestos or lead
WORKING WITH ASBESTOS, OR UNDERTAKING A JOB WHERE
THERE IS A LIKELIHOOD THAT ASBESTOS IS PRESENT IS
A HIGH RISK TASK FOR WHICH A JSA FORM MUST BE
FILLED OUT PRIOR TO COMMENCEMENT OF WORK.

COMMENCEMENT OF WORKS.
WORKING AT HEIGHTS -Jobs that require being more
than 2 metres high (from the ground to your
feet)
WORKING NEAR POWER LINES -Jobs that require us to
work closer than 6 metres to a main power line
on the street. And Closer than 2 metres to a
private power line to a private house.
GAS LINES -Jobs that require us to work near gas
lines, or with gas lines. Natural or LPG
DEMOLITION -Jobs that require us to carry out
demolition works, stripping walls Pull off roofs,
excavate concrete.

  • Using an extension ladder
  • Tie the ladder off top and bottom
  • Falling
  • Ladder Slipping
  • Wear your protective boots with grip (see your
    PPE)
  • Use an approved ladder
  • Have 3 points of contact on the ladder at all
    times
  • Feet of the ladder in good order
  • Make sure the ladder goes 900mm over the top of
    the roof, unless through a manhole
