  • An Alberta Health Wellness Initiative
    Coordinated by David Thompson Health Region and
    Chinook Health Region to build capacity to
    deliver chronic care self-management workshops
    by
  • training new leaders
  • training new master trainers
  • supporting the development of new regional
    infrastructures

Why are we talking about Self-Management?
  • Up to 60 of Family Practice care is related to
    chronic conditions, and they lead to 70 of
    health expenditures
  • Chronic conditions require a different approach
    than acute, episodic care
  • Self-Management workshops have been shown to
    complement clinical services for chronic
    conditions

Self Management
  • Purpose of the self-management program is to
  • help patients manage their life with chronic
    disease
  • to increase patients skills and self-confidence
  • to problem solve and make decisions
  • Patient self-management is a critical component
    of Chronic Disease Management

Chronic Disease Self-Management Program
  • Self-management programs, developed by the
    Stanford University Patient Education Research
    Center
  • In existence for over 24 years
  • Programs offered worldwide

Living a Healthy Life with Chronic Conditions
  • Increases the skills necessary for individuals to
    manage chronic conditions and work effectively
    with health care professionals
  • Designed to enhance regular treatment and will
    not interfere with existing programs or treatment

Workshop details
  • Workshops are highly interactive, focusing on
    building skills, sharing experiences and support
  • Helps individuals take responsibility for their
    care
  • Program is led by volunteer leaders with chronic
    conditions themselves

  • Workshops are 2 ½ hours weekly for six weeks
  • Topics covered include
  • Developing a long term exercise program
  • Fatigue management
  • Healthy eating
  • Communicating with family and physicians
  • Developing decision making and problem solving
    skills

Access at the local level
  • Options
  • Referral to RHA chronic disease program that
    combines self-management, disease/risk education,
    exercise rehab
  • Referral to specific workshop run by RHA or PCN
  • Registration through a workshop offered by a
    community agency

Results
  • Based on 5 year research through randomized,
    controlled trials at Stanford
  • - Fewer hospitalizations (0.8 days during first
    6 months)
  • Improved communication with physicians
  • More confidence in ability to manage their
    condition
  • Less fatigue
  • Less disability
  • Improved function
  • Increased minutes of exercise
  • Increased use of stress management techniques

    T-Trainers from Stanford

    leader training is to
  • build resources expertise in all PCNs and
    RHAs
  • ensure comprehensive province-wide support of
    chronic disease self-management programs

  • Majority of the grant for direct delivery of
    training
  • Designed to meet needs of PCN populations.
  • Enable the widest possible access to training to
    all health regions
  • Gather input to the program by means of an
    advisory committee

Key Principles

  • PCNs and RHAs will utilize the training to
    provide the service to their clients/residents
  • Training to be provided at no cost to PCNs,
    RHAs or lay persons in Alberta
  • Methods for training can be flexible depending on
    the needs and resources of individual health
    regions

  • Provincial coordination / management
  • Stanford licenses for RHAs
  • Remuneration or backfill for Trainers
  • Transportation and lodging for trainers to
    provide training

    rental, etc.)
