What is the RGP and what is it really called?

• 5 RGPs in Ontario
• we are SERGP!!
• catchment area is KFLA, LA, HPE, LLG counties!!
• Focus on high-risk or frail elderly
• usually gt65 years old
• complex problems requiring comprehensive
  geriatric assessment

• Outreach
• Consult Team
• Gibson 3 in-patient unit
• Day Hospital
• Clinics (SMOL satellite)

What does Outreach do?

• Gatekeeper role for RGP programs
• interdisciplinary assessment in patients home
  environment
• make recommendations about environment,
  diagnosis, community resources

What happens on Gibson 3?

• Currently 16 beds for assessment treatment
• focus on geriatric rehabilitation
• average length of stay 4-6 weeks
• 50 of admissions from KGH
• 40 from community

What do we mean by Day Hospital

• multidisciplinary team assessment and treatment
  in an outpatient setting
• attend 2 days/week
• average LOS 8 weeks

• team includes MD, CNS, OT, PT, SW
• KGH staff focus is on rehab potential
• we also focus on geriatric management issues
