Why TIARA?
  • TIARA was designed by a diabetes educator to
    simplify the data collection for program
    recognition.
  • Program recognition allows for Medicare
    reimbursement for this important activity.
  • Manual collection of program statistics for
    recognition takes hundreds of hours.
  • TIARA software is extremely simple to use and
    allows the educator to focus on patients, NOT on
    data collection.

The TIARA Screens are divided into two sections
  • The left side has four tabs
  • Patients
  • Physicians
  • Classes
  • Reports
  • As you select each tab, the right screen shows
    the required fields.

Patient Demographics
  • The Patient Status field allows the educator to
    track whether patients are in the
    comprehensive/initial phase, or the follow-up
    phase of the program.
  • Other demographic information is entered, using
    free text. Pull-down menus are available for many
    fields.
  • The referral physician is selected from a
    pull-down menu of physicians already entered into
    the data base.
  • Special needs (hearing or sight impaired, etc.)
    are selected as appropriate.

Patient Assessment
  • The ten content areas from the 6th Edition
    National Standards for DSME are listed in a
    pull-down menu.
  • The educator selects each content area in turn
    and decides if it is applicable to the patient.
  • The educator then indicates whether group and/or
    individual education is needed and whether a
    behavior change is indicated.
  • As required by the 6th Edition National
    Standards, there is a field to note cultural
    beliefs, financial issues, etc. that may impact
    the learning process.

Patient Education
  • Group classes are assigned from a pull-down menu
    of the classes already entered into the program.
  • Individual education is documented using a
    pull-down menu of the content areas.

Patient Behavior
  • Individual goals are developed by the patient
    with help from the educator.
  • Goals are categorized using a pull-down menu of
    the ADA content areas. Later, this information
    can be used for outcomes research.
  • Individual goals are documented.

Patient Outcomes
This screen provides space to document the
outcomes your program follows, including
  • - Blood pressure - Satisfaction with care
  • - Body mass index - Quality of life
  • - Lipid profile - Eye Exam
  • - HgbA1C - Foot Exam
  • Microalbuminuria - Smoking cessation

Two additional outcomes can be tracked, if the
user opts to define the Measure A and Measure B
fields.

Individual Patient Reports
  • Individual Patient Reports are generated by
    right clicking on the patient name.
  • Once selected, a list of reports is displayed
    from which you have the following options
    Demographics, Assessment Summary, Education
    Summary, Outcome Summary, Behavioral Contract,
    and Provider Follow-up Letters.

Behavioral Contract
This report is provided for the patient to help
track his/her progress in meeting behavioral
change goals

Physician Follow-up Letter
  • This letter to the referring physician outlines
    the educators interview with the patient.
  • The physicians address, reason for referral and
    patient name are already inserted into the
    letter.
  • This letter can also be edited.

Program Reports
  • Program reports are available by selecting the
    Reports tab on the left screen.
  • Several of the reports are duplicates of the
    American Diabetes Association program recognition
    application.
  • Other reports are for use in outcomes research
    and continuous quality improvement initiatives.

  • Our toll free number (877) 472-4338 is available
    Monday through Friday 830 am until 530 pm

TIARA on the Internet
  • The Web/Internet version is the only product now
    being offered.
  • The Internet version allows access by any
    educator from any computer or location with web
    access.
  • The Internet version has security features that
    support compliance with HIPAA regulations.
  • Updates are normally made twice per year with
    enhancements and bug fixes.
  • Changes to support the 7th Edition National
    Standards will be included as needed
